What's a good 3rd party tool for creating MSI installers?

I've been using Visual Studio Setup projects to create my application
installers but Setup projects seem to be a tad limited. What's a good
alternative? I'd prefer something that doesn't cost an arm and a leg like
InstallShield, and I assume there must be something out there. Any
suggestions would be appreciated.
