Add custom php page in wordpress

Hi friends
I want to add custom php page in wordpress..
I have created 2 pages. One is simple form and other is submit form.
i already added simple form code in header part which i require.
now if i paste submit form(submit.php ) paste in my theme. but its not work...
i attached 2 pages which i created in core php.
Please help me what i do?

I created a report to print labels, and it created a custom page size for the report. When I opened the properties of the printer driver I could see it listed - as Custom. Then I had to change printer drivers. (Long story, rather OT.) When I displayed the report, it was formatted for a full page - not real useful for mailing labels. Hello. If my users are logged in, and try to access restricted pages I want to direct them to a custom 403 page. If they are not logged in, I would like to continue to direct them to the login page as is currently happening.
