problem adding second input button

I've had a garden calendar posted with lots of users over the past 5 years. (http://www.kjmbiosyste ms.com/Skippysgarden/planting%20cale ndar.html) BYTES helped me with this - thanks! This calendar has a single input date and from this calculates a dozen other dates.

I am now trying to update this calendar to collect 2 input dates and use them to calculate about 2 dozen dates.

The code is not working. Can someone tell me how to fix it?
