How vb 6.0 forms working with dreamweaver 8.0 for build web pages in xhtml

I have a problem working with VB as a form for build web pages in dreamweaver and use Xhtml. I want to know how to take this form ( VB 6.0 ) to dreamweaver as a forms

Considering how old VB6 is, perhaps there isn't any way to take it across.
