See if extension is NOT associated to an icon

All,

Couple of questions:

1. I am using the SHGetFileInfo() function that has been posted several
times on the forum to get the icon associated with an extension. Is there a
way to find out if an association does NOT exist? This function will return
a generic default icon if there's not an association, but how can I
determine that in code?

2. If there's not an association, how can I create one from within my
application?

I want to be able to get the icon associated with a certain file extension. I can read the registry hkey_classes to get the ext. then look to the other key for default icon. How can I then extract the icon in file x at index 1 or whatever? Again, I am not dealing with a file exactly but with a string path of the file that has an extension.
