See if icon is NOT associated to an icon

All,

Couple of questions:

1. I am using the SHGetFileInfo() function that has been posted several
times on the forum to get the icon associated with an extension. Is there a
way to find out if an association does NOT exist? This function will return
a generic default icon if there's not an association, but how can I
determine that in code?

2. If there's not an association, how can I create one from within my
application?
