how to make Quiz in php

Hello Everyone:
I'm looking for a template or a website that will help me make/build a quiz.
The quiz needs the following:
1. T or F, mulit. questions, click and drag answers.
2. Client must register to take the test.also holds all results after taking the test.
3. Client must sign in to take the test
4. Quiz gives final score and a pass or fail

I know some PHP, SQL and XML.

Sorry but this is kind of not a 1 post deal. It took me a month to design a forum and make it secure. Just a bit more advanced than a little quiz, but nobody can do this for you unless you like collaborated. Although I can give you some tips.
Check out http://php-sec.org/ they will teach you about having secure logins.
Get "Learning PHP 5" by O'reilly
Learn AJAX, it will make the test more responsive...

i made one in 2 days, its really simple and no security issues were fixed.. If you want i can send u a copy. But lemme be clear, it is not organized, but its works though :p

I made it in a flash of time for my college.. :)
