Linking PDF forms with SQL

Does any one know how would you link PDF form fields with SQL database
using ASP perl?

I want to extract the data from the PDF form and insert it into the
database and vice versa. I read a lot about using FDF (File data
format) but I am having a hard time implementing it.

Would really appreciate any suggestions you may have.
