This is not very complicated.

Right click on your project in solution explorer and choose Add->WindowsForm.
Design the form the way you like/need it.

When you need to use it, you declare an object of the type you created and display the form using the Show or ShowDialog if you want to behave like the predefined dialog boxes.

Any windows form can act like a dialogbox. Instead of using .Show(), use .ShowDialog().
You have other things to do like setting the .DialogResult for your form based on user actions, but that is very simple.

What Modal class?
If you are asking can you inherint the openfiledialog class then the short answer is "no" the long answer is "yes, if you find a custom .dll made in c++ and import it"
If you are just looking to make a popup dialog, you can inherit from the sysyem.windows. forms.form class
