Hey guys , am just getting into the AJAX scene and am a noob.
I am really worried about Ajax upload , cant figure it out.
I have a basic html form , like this

This form uploads a given file to the server where upload.php is located,
I need to convert this to ajax ,
like I need to send those information and file to the server with Ajax method ,
so that I can show the progress of my file , success message and reset (which resets the form so that one can upload a new file again).
Please help me with these.
Remember:
1. Information must be sent with the file to the server
2. Progress bar is needed
3. Success message is needed
4. Reset form option is needed so that one can upload again
THIS IS VERY URGENT , AM NOT ABLE TO FIGURE OUT , PLEASE HELP ME. THANKS.

Removed "help urgently needed" from the title which (believe it or not) makes the problem less urgent in other people's eyes and less likely to be answered.
Finally, to your problem. See this earlier thread. You'll need to use an iframe for this which will allow you to upload a file without refreshing/reloading/unloading the entire page.

with firefox and safari it is no problem at all. only IE makes some
problems.
my script is based on ajax-uploader, which can be found at
www.srmiles.com/freestuff/ajax_file_uploader/ . you can do multiple
file uploads. each upload will have it's own "form"-tag, so that each
file is uploaded for its own. could be a good solution if there are
"big" uploads.

which supposedly allows you to upload files using AJAX (I'm not an
expert). The site claims it's the best way to upload files out there,
and the file size is only 1 kb.
I couldn't get it to work for me. I downloaded the files to my
computer, and opened the index file with my browser. It doesn't work
