Email disribution list using GoLive

:confused:

I am currently working in GoLive and need to know how to set up a distribution list. What I need to happen is, people enter their email address and hit the submit button. When doing so, their email address gets sent directly to another recipient's email address. This person will keep a list of people unsubcribing to this list.

How do I make this happen? I'm incredibly rusty when it comes to scripts and codes, so please go slow with me. Thank you!!
