Deployment & Local Debugging

I'm having a hard time figuring out how to structure my development
environment for web services and a database. I have a remote server
with sql server 2005 on it. I'm building an application which uses
windows forms as web service clients and the web service acts as a
proxy to the database.

My problem is, how the hell do people normally set things up so that
to work with databases. Do they normally use one database for both
development and deployment? If they use two, how do they make sure any
changes made during development are made on the server? And how do you
do that easily. Other alternatives are to allow the sql server to be
accessed remotely or to first deploy the web services to the web.
