Automatic header comments in .cs files

Hello
Does anybody know a tool/macro with can automatically add a header
comment to a .cs file at saving it ?
I want something like this:

eg.
///Automatic comment
//CREATED BY: klimmass
//MODIFIED BY: klimmass
//MODIFICATION DATE: DateTime.Now()
