Essential Mathematics for Games and Interactive Applications

I just bought it on a lark. After looking it over, I have to say it looks
to be well written. The reason I'm mentioning it here is because it comes
with what looks to be a pretty nice C++ library. The licensing terms for
the CD seem quite reasonable. Basically, you can use the code for your own
projects, but don't act like you wrote it.
