I am trying to find a link to the 'C++ Annotations' by Frank Brokken (I
think Version 5.2.4 is the latest) that works because I've been unable to
find one that does. Can anyone suggest anything?

file_list table has a unique field called file_name.
The problem I am having is that if no unique items exist, this will
insert into the table fine. But if a unique item does exist it will not

to work ?
Are they part of the new MS C language (that's, real tokens understood by
the compiler), or always #define'd as nothing and interpreted by some
external tools?

It sounds to me like ASP, not ASP.NET is handling the
request for WebForm1.aspx. This is most likely an IIS
config issue that may have been caused by order of
installation or reinstallation, or possibly even
something that happened in IIS before .NET was
installed.

I am unable to find appropriate documentation on the type promotion of bit-fields when using the generalised comparison operator "<=>".
The problem is that using the GNU compilers, it seems that the internal comparison operator "<=>" tries to promote arguments from unsigned to signed.
