DataBase Login screen shows when running Windows form with Crystal Report depend upon dataset in C#

Hi All,
I had problem when using Crystal Report in C# windows application.
I create report depend upon dataset using table from SQL 2000 and add
it in a C# windows application.
NP: The application had 2 forms, one with preview button which will
call the report from the other form and the other hold the crystal
report.
The time I get to run the application and preview the report by
clicking the preview button from the first form I get Database Login
screen asked me for the coming information.
1) Server Name
2) Database
3) Login Id:
4) Password:

I didn't know where this screen comes from.
And I search so much to be able to cancel this screen.
I appreciate anyone help. Thanks for everyone help.
