The bold tag could be handled in a class attribute, but I need a <br/>
tag inside the form for formatting and the input fields are necessary for
getting data.
Error 3 Validation (XHTML 1.1): Element 'br' cannot be nested within
element 'form'.
Error 7 Validation (XHTML 1.1): Element 'b' cannot be nested within
element 'form'.
Error 9 Validation (XHTML 1.1): Element 'input' cannot be nested within
