Anyone know of a good “Registration / Join In” request script

Hi,

I’m looking for a Registration / Join In script in either asp.net or asp.
User will fill the form (First Name, Last Name, Email, Company, Phone, etc)
and after submitting the form, an email will be sent to administrators who
will decide whether to grant /deny request for access. Upon denying or
granting, email will be sent back to user informing him/her that he or she
has been denied or granted the access.

Because of the time crunch, I am looking for a script that would do these
things. I can do the little modification if necessary. Preferably such script
should be using MS Access or MSDE. I would appreciate if anyone knows of any
such script or tool, please let me know.

its only a handful of code lines.........

Create a script that has some form fields and a method that accepts those
values and creates an html email bound for the admin, with the values in and
a hyperlink to another script in the mail.

The hyperlink contains the querystring params to do a db insert on it being
clicked with the destination page doing the insert and mail to the user in
the onload method.
