Did it disappear altogether or just when displayed? If it disappeared altogether, then you need to add it back. In Form Design View, go to the Form Design Tools Ribbon, in the Show/Hide Group, click on Form Header/Footer and it should add an empty header/footer.

If it only disappeared when displayed, then check the Visible Property of the Header/Footer by clicking on it and viewing its properties. It could also be that the Display When property is set to Print Only.

Let me know if this works.
