Form programs allowing output to look like the orignial form.

Im looking for a Form Program that allows someone to easily create a
form (ex: word or frontpage) then publish it to the web. Then once
someone hits the website they can fill out the form and the
information can be saved to a database or emailed to the original
maker of the form. Then someone can open and view the form in the
same view as the customer sees after they input the information into
the field.
Pretty much so that the Form inputed looks exactly the same as the
output form viewed by the creator.

Microsoft Front Page may do what you want. It's commercial software, not
freeware. But my question would be "why ask for web stuff in a newsgroup
devoted to Microsoft Access database software?"

Front Page will do some but It wont allow the output.

I asked this here because I wanted to know if anyone had ever done
anything like that with a program and then integrating access to save
the input. But like i said front page will work to create the form
but you will have to know scripting to save the fields to a database
such as access. Front page will not send the form in its original
state to the creator. It will send the results but not in the
original look.
