How to show all main and sub categories in PHP script?

I have set and corrected http video stream scripts in php
Only one thing I am unable to set I am interested to show all main and sub categories on all pages enable visitors to click on them and move on that categories pages at the bottom before stats all category names and its sub categories like if A is main category and A1, A2, A3…… are its sub categories then script show it like that:
A: A1 A2 A3 …………
I have tried to set it but it is showing only one main category image in place of its name and only one sub category you can see the page: http://www.hazratsultanbahu.com/comm...e=Video_Stream

Both script files are attached "layout" and include "JavaScript" files in ZIP (EDiting Vdeio stream.zip) format you can see
Required to add in the line of /// required to add

I shall be thank full if any only help me
