Tutorial for MySQL questionnaire

Hi all,

I am looking to build an online questionnaire which takes values from
an HTML form and stores results in a MySQL database.

Can anyone recommend a good tutorial for this?

Build Your Own Database Driven Website Using PHP and MySQL, have found
this book very handy,

Have a look at the source code for something like Moodle (online
classroom software) or Drupal (a content-management system with built-in
questionaire/polling capability).

Both are free, downloadable code, PHP and MySQL-based, with plenty of
documentation and user support, including books you can order from
Amazon and read at leasure, if you want to go that route.
