survey sample application....

Hi,

I downloaded Survey Admin sample application from MSDN.
Works good.
Now, I am trying to find sample of presenting survey and saving data in SQL
Server Survey db. Looks like MS removed this example from their site.
Please, help me to find this sample application.

I downloaded and install both the Survey Sample and the Survey Admin pieces. Installed both of them and everything works. The problem I'm having is when I fill out the test survey nothing gets saved back to the SQL database. It saves a new session number in the session table everytime, but never puts anything the the response table. I'm not getting an error and the surveyform.aspx gives me the "Thank you" message like it was stored ok. ...
