PDF forms -- anyone have workflow success?

hello,

i am curious if anyone has had success w/ a new workflow my group is
discussion. the proposed flow:
- our users, when online w/i our firewall, will request the intranet
webapp to produce a PDF. this PDF is a form (and probably dynamic based
on user)

- the user then takes it w/ them as they travel outside the firewall,
offline w/ a laptop.

- after completing a job, the user will fill-out the PDF form. if form
is valid, it can be saved

- the user then returns home, w/i the firewall and uploads the PDF into
the application

- the application will parse out their form-input values, and insert
into the database

- the application will then archive the PDF into the database for
future historic referral.
....has anyone done such a thing? successes? what
form-creation/validation techniques did you employ? which components
did you work with? thoughts?
