Tab (page) with Dynamic Questions

I am working on a survey. All survey's will have five tabs (pages) that have the same types of questions. The questions fall under these five categories.
Question Category
Business / Billing
Economic Advantage
Quality
Support
User Satisfaction
There will be different sets of questions for each categories based on the vendor name selected.

How can I set up the tab/pages to dynamically populate the questions associated to the selected vendor? I will also be setting up radio buttons for each question that will have values for:
Does Not Meets (1)
Low Meets (2)
Meets (3)
High Meets (4)
Exceeds (5)
Would I need to use a label for this or a text box?

Any input is appreciated!

The best approach will be to use a "mainform" with a combo for the Vendor and a combo or listbox (single select) for the categories.
Now place the subform based on all questions and after placing single-click the subform to open the properties and get the Linkage master and child fields under the datatab. For the "Master" fill in the name of the Vendorcombo and the Categorylist (separated by comma's) and for the "Child" the fieldnames for Vendor and Category on the subform.

Now Access will sync the mainform choices with the subform.
