help with access

I have created an unobound form in access, which is survey form, with 20 questions, with 5 check boxes, only one answer can be picked for one question, then there is comments for each question. I want to store the user input in a table, which i created later, table has QID, Questions, Ratings and comments field. I want the user input to be stored in this table in the ratings and comments field. Any help is appreciated.

Create a command button to save the results (don't use the wizard) and in the On Click event of the button open a recordset of the new table. If you are not sure how to do this have a look at the following site.
