Creating a survey to be completed multiple times for the same parent record

I am trying to create an Access database to track surveys completed multiple times based on a set time sequence (6 months, 12 months, 24 months) by the same person. I am completely lost as to how to set this up. Can anyone help or point me in the right direction as to where to learn how to best design this model?

I think you should start with some of the online tutorials. Open google, and type "Access Tutorial". There are a lot of free ones.

I have done that it still did not give me enough to figure out the history storage. If you recommend something specific that would be great. I have everything about this figure out but being able to use one survery over and over and compare response changes from the record owner.
And in future please try to make sure you use the Reply button and not the Report button
