Use string concatenation:
string & string & string
Your query would look something like this:
SELECT home_address & " " & school_address & " " & courthouse_address &
" " & doctor_address AS AllAddresses
FROM ... etc. ...
If you want new lines after each address you could do this:
SELECT home_address & chr(13) & chr(10) & school_address & chr(13) &
chr(10) ... etc. ...
