Right above a procedure.

This feature used to work but now it does not. I enter '''
on a blank line above a procedure/function/declaration..anything, and the
XML Comment Template does not appear. Is there a setting or something that
might have been turned off?

How?

I can confirm that this worked and then stopped to work randomly. I
tried various settings without success. Then I imported VB settings and
it started to work in .vb files but only in Web projects. It still
doesn't work with WinForms VB projects. There also no "Insert Comment"
in context menu.
