Custom fields

I'm writing the scope of an app that will be built in
VB.Net, using Access at first. One of the requirements
is to have user-defined fields for particular tables.
What is the best way to provide that functionality? One
way would be to provide a few extra fields, say 5, at the
end of each table, which the user could do whatever
with. Another method is to have a table that stores an
unlimited range of custom fields as named-value pairs.
Any ideas? Thanks!
