Can anyone tell me how to take the information input from a simple web form
(with single selection drop down selection boxes) and include that in the
followig query?

You need to learn something about programming, mysql isn't going to help
you. Go take a look at some of the free web script sites that are all
over the web, and that should at least get you started. Either that or
you can find a free ready-made script to do what you want to do.
cgi-resources.com, bignosebird.com.... etc, there's lots of places to
find what you need.

In what environment? Perl? PHP? ASP? JSP? J2EE? ASP.NET?
There are a lot of choices. And it's off-topic for this group - it has
nothing to do with MySQL.

If you're running on a linux/unix type platform, then the simplest and
easiest to get into immediately (that will usually make the most sense
imo - but I'd get blasted for saying this), is probably a perl/cgi
script. If you want something with a little more pizazz that's a bit
more forward thinking, go with PHP.
If you're running on a Windoze server, then the asp thingy is probably
the way to go.
