Printing Paper Forms

1 New Member
I am building an app that requires printing of multiple paper forms for each record in a csv text file (could easily be MS Access or mySQL table rows). Each row in the csv file represents a patient being admitted for a surgical procedure. For each patient, I need to print 18 different paper forms which comprise that patient's "chart". Each paper form needs to be pre-populated with some data from the input file (e.g, name, age, etc.) before printing (so can't be a static pdf file for example).

Each paper form is complex with dozens of fields and checkboxes.

This will be phase 1 in an eventual conversion to paperless office environment. For now, looking for a way to automate generation of the patient "chart". Was experimenting with a Java front-end app combined with Omniform, but there doesn't appear to be a way to programmatically initiate printing of a form from an external app.

Can anyone suggest a forms management tool that allows creation of complex form "templates" that can then be populated with data and printed via an api?
