Calling a form from a Query in Expression Builder

Greetings all. I am trying to call a form from expression builder in design view of a query. The target form has one field, a list box that let's the user select a category, and then click the OK button to get the results of the query based on this selection. I am having difficulty getting this to occur.

I made the small form just for the sole purpose of the selection criteria on the query. Am I going about this the wrong way, or is their a simpler way to accomplish my goal? Thanks in advance.

You might want to try a ComboBox instead of a listbox. I believe the combo is easier to work with.
