Create a query, adding the 3 tables concerned. Select the fields you want on the form annd save the query.
Open the forms tab, select New, select Autoform: Tabular, Columnar or Datasheet, depending on which type you want. Select your query as source and save the form

If you want to update from the form make sure you have selected "Enforce Relationship Integrity" AND "Cascade Update Related Fields" when creating your relationships.
