There can be a number of reasons for that, which is hard to decide
based on the minimum of information you've provided for us, but one of
them is that there might be an an error with your network connection.

Another frequent problem causing similar error messages, is when there
is corruption in the db, which I've been fortunate enough to be able to
solve through using the /decompile option.

Here is a read on corruption
http://www.granite.ab.ca/access/corruptmdbs.htm

Here is a step by step instruction on usage of the /decompile option
http://www.granite.ab.ca/access/decompile.htm

I don't think it would hurt to try the /decompile option on a *copy* of
your db.

Roy's suggestions are logical. Is the database you created your new
form in, or the table you based your form on, located on another
computer? You may need to verify that those files exist and that your
network rights allow you to read from them. Also, as is obvious, check
your physical connection to the network.
