Utter newbie question re simple links

Hi! Am a total newbie to XML and have been asked to update a questionnaire template that was created last year with new information for this year. I've got my head around most things now but am utterly stumped as to how to add a URL! I have a link to another website that I want to add to a simple paragraph and, if possible, for this link to open the new website in another window. This part of the document is not nested in anyway and is just straight forward text: <p>my text is here</p>. I presume I need to link or something but have no idea how to go about it. I've tried googling for help but anything I found was way over my head or was for images etc.

Any advice would be much appreciated!
