Word macro for using access databases

At work I have a form that we enter data from access into, and then fax out. I have to base any code I do off of the form itself, so I cannot create the form from access.

What I want to do is have a form open at the launch of the file, with a combo box to choose the hospital state and one for the hospital name. The hopsital name is under seperate tables, each state has a table for its hospitals. I have already created the form itself, with the combo boxes, etc. And I have created form fields to fill in the other information, like the phone #s, fax #s, etc. However, I cant figure out how to link up the database in the macro.

If I can get this done, it will save me soo much time, but all I can find is how to create a word doc via access.
