Question numbers

Can anyone tell me how to have a question number generating from 1 to
60 which I will display questions from the db and the question number
will be there beside it? I would also like to know how I could keep
track of the user who logged in and keep a "frequency" of the number
of times he/she took the test?

Much appreciated.
