I recall doing this about a year ago, I remember seeing the screens shown
here:
http://www.c-sharpcorner.com/UploadF...ceAccount.aspx,
especially the "Custom actions." I had to add something to each one of
them, not sure if it was Primary output or not, but I can not figure out how
to get to these screens!

I've right clicked everywhere I can think to right click and "Installer"
never appears on my context menu under "Add"....
