Selecting a File from Various versions

Hello,

I was wondering how to I create a web page in which a user can select
what version of a form he wants. For example, if he fills out a form,
I would like for the user to be able to save it. If the user revises
the form and saves the revised form as a different file, I would like
him to be able to select which version of the form he would like to
use and submit. For example, if a user has Form1, Form2, and Form3
(same form, just three different revisions), I would like for him to
be able to see all three forms on the web page and select which one he
wants to use. Does anyone have any suggestions? Thanks for all of
your help.
