Publish in 2005 Express

I've created a small app which uses an access database.
When I click Build Publish and choose publish to file eg. CD / DVD it
creates a setup file (installer) which for some reason puts the executable
in the start menu (not a shortcut) when run and doesn't deploy the database
file even though it is part of the project.

What am I doing wrong? The few options given by the publish wizard only
choose publish file location etc. and don't make any difference to the final
output.
