Readme file + xml list of updates

I want to include a readme file + xml list of updates to this version
with my VB.NET setup.
I have a number of questions:

How do you add a readme to your source code and get the visual studio
setup program to place it in the program directory when the setup is
run (i'm using a standard visual studio set up project)?
Is there any recommended way of listing program changes in an xml
document.
Is there any recommended standards for your readme file
