AddForm to print custom pagesize

I am an delphi programmer trying to convert to vb.net.
I am stuck on something that is quite simple in delphi, but I can not
find any code to do it in vb.net.

I need to add a form to a printer to be able to set custom page sizes
on the fly.
After the user has printed using the custom form, I also need to know
how to delete the form.

I can find lots of people inquiring on how to do this, but no answers
or sample code.

Yes, I am using Windows 2000 and XP.

I know the printer support the forms and I have the permissions,
because we do this in delphi everyday.

Can someone please help me figure out how to addforms and deleteforms
in vb.net.
