SQL 2005 - Disappearing Relationships in DB Diagram

Relationships between tables disappear without explanation in the
database diagram using SQL Server 2005.

More significantly, are the foreign key constraints really disappearing
or are you just seeing a bug with the UI? Check the constraints.

Can you describe any steps to reproduce the error? You can report a bug
at the feedback centre:

Personally I wouldn't touch the diagram tool for anything. It's
certainly no good place to make changes to tables and keys.

Whenever possible please post enough code to reproduce your problem.
Including CREATE TABLE and INSERT statements usually helps.
State what version of SQL Server you are using and specify the content
of any error messages.

If you press save for some table changes that fails half way, you can
indeed lose constraints and foreign keys. There are several severe bugs
with the Table Designer in Mgmt Studio. If you ever use it to develop
changes, never press Save, but always generate change scripts and
review these carefully.
