MS SQL Server 2000 Log Shipping problem

We have successfully implemented Log Shipping between two MS SQL Server
2000 servers (bothe SP3). The problem we are having is two fold:
1) The tran log on the primary server is not being truncated even
though the tran logs are being shipped and applied to the standby
database on the backup server.
2) The log shipping monitor, when drilling down into the "View
Copy/Restore History" reporting selection, is showing a copy failure for
"first_file_000000000000.trn". The reason given for the failure is:

"The process cannot access the file because it is being used by another
process."

This is the second time we set up log shipping between these two
servers. We deleted the first log shipping set up to work on a problem
we had with logins not populating correctly. The second log shipping
build went along with no errors indicated, however, the above two
problems do exist. Anyone ever experience this scenario before?

I appreciate your insights.
