document table schema

Dear all,

I am new to SQL programming.

I'd like to know what are the usual practices in documenting the table

How to document relationship among tables? (by diagram or text description?)
If I need to draw the schema in diagram, which software should I use? (Dia?
any better tools?)

Is there any articles teaching people how to do documentation for database?
