Some time data inserted in database and some time not

Hello,
I developed a project and i used PHP , AngularJS, JavaScript, Jquery, Mysql technology. When any user want to register then he/she has to fill all 4 steps after that user is registered. when user is register then sometime all data inserted into database, sometime only step 1st data is inserted but 2nd and 3rd step data is not inserted properly. It happened only some time not every-time and user dose not get any error message. After registration if i am trying to insert user's remaining information through admin panel that time also data can not be inserted and am not getting any error message.

Why this happened please help me.

Why this happened
you’re not using SQL transactions and you’re missing input validation/database error handling.

with this much information that’s pretty much all we can say.
