Inserting a row in mysql database using multiple forms

I would like you to help me know how to insert a record in mysql database using different forms

- How does your database look like?
- How do the forms look like?
- What information is mandatory?
- What are your trying to accomplish?

The answer to the last question is obviously "insert a record", but i'm asking because why do you want to do it 'using different forms'....
