Undefined Index Error

Please help me, I keep getting "Undefined Index Error". I'm also a noob in PHP. I can't echo back my equipcode and adddata, though I can successfully echo back curloc. Curloc and equipcode are basically the same codes. The others are working fine. Following are links to my codes (text-based):
