Suggestions for PHP backoffice CRM

I'm looking for an open source backoffice or CRM written in PHP.

Ideally, I want to have my website form tie directly into the CRM. This way if someone fiils out the form, it gets entered into the backoffice automatically.

Anyone have any suggestions for me?

Not sure if SugarCRM does what you need, but it's a very good CRM. Look into its features to see if it does that. Probably does.
